Thrive Kids Day Camp Registration

Thrive Kids day camp is a one day camp for kids in prep - year 8. The goal of the camp is for kids to have fun, make friends and grow in their relationship with God.

The camp will run from 10 am - 4 pm on the 26/09/2020. Your child will need to bring their own lunch, water bottle, hat, enclosed shoes and make sure to wear clothes you don't mind getting dirty.

PHOTOGRAPHY & VIDEO: Thrive Kids services include photography and video recording. Images of people attending or participating in
Thrive Kids may be used in church publications, church buildings, website or social media channels. As a precaution, your child’s name
will not be published or linked with photographs. By attending the activities of Thrive Kids, you agree that we may use your image as mentioned above. If you have any concerns please speak to a Thrive Kids leader.

Agreement

In the event of illness/accident, I authorise Thrive/New Hope Church, its staff and representatives to obtain whatever medical assistance is deemed necessary and agree to meet any expenses attached to such treatment. I further authorise qualified medical practitioners to administer medications, blood transfusions and anaesthetic if such an eventuality arises.

I understand that Enrolment details are available to Thrive/New Hope Church staff and leaders in the programs in which my child is enrolled.

I understand that it is my responsibility to advise the staff if I do not wish my child to participate in a particular activity.
